Out-of-story, because Cai was Arthur's original top knight, and as each
new writer created a new bestest-knight-ever, poor Cai/Kay got pushed
further and further toward the bottom of the heap.

In-story, because Kay's mother breast-fed Arthur instead of Kay, who had
to settle for an (ugh!) peasant woman, and consequently picked up all
that nasty peasant vulgarity from her.

It still doesn't explain why he insults the likes of Sir Gareth, La Cote Mal

I think it was Phyllis Karr who pointed out that as seneschal Kay would
have been in charge of all the lordlets who were dropped off in Arthur's
household as diplomatic hostages and needed to be trained as knights. He
was their drill sargeant. It was his job to be nasty to them.

Seneschal and butler are not the same thing. Bedivere's brother Lucan was
Arthur's butler. I may not know exactly what the difference is, but if
there wasn't a difference Arthur wouldn't've had one of each.
